SIP Construction & Operating Permit Application

The Georgia SIP Construction & Operating Permit Application (SIP Application) is used to apply for initial air quality permits and for modifications to existing air quality permits. Please note: a Title V permitted facility that is requesting a modification involving construction must complete both the SIP Application and the Title V Application.

If the facility is requesting a name change or an ownership change and no other modification, go directly to Additional SIP Permitting Information below.

The SIP Application consists of separate forms that request information about the facility, emission units, control devices, and pollutants emitted. Each application form contains specific instructions for completion, and the individual forms may be downloaded by clicking the form name listed below.

The Application Submittal Document contains information for submitting a SIP Application Package, including the mailing address.

A complete SIP Application Package consists of Form 1.00 General Information and only the necessary application forms for the permitting action requested. The SIP Application Package should include all facility diagrams, maps, equipment drawings, calculations, and other attachments. However, do not include any blank forms or form instructions.

Form 1.00 General Information
Must be completed and submitted with ALL Georgia SIP Air Permit Applications, along with any other relevant forms listed below.

Emission Unit Forms

Form 2.00 Emission Unit List
Must be included if requesting the construction, operation, and/or modification of any emission unit.

Form 2.01 Boilers and Other Fuel Burning Equipment
Must be included if requesting the construction, operation, and/or modification of any fuel burning equipment (boiler, burner, engine, turbine, dryer, smelter, etc.) excluding incinerators.

Form 2.02 Organic Compound Storage Tanks
Must be included if requesting the construction, operation, and/or modification of any organic compound storage tank greater than 4000 gallons.

Form 2.03 Printing Operations
Must be included if requesting the construction, operation, and/or modification of a printing press.

Form 2.04 Surface Coating operations
Must be included if requesting the construction, operation, and/or modification of any surface coating process, excluding Printing Operations.

Form 2.05 Waste Incinerators
Must be included if requesting the construction, operation, and/or modification of a solid/liquid waste destruction operation. For additional reference material on determining the type of waste combusted in the incinerator, please refer to the Waste Classification Guide.

Must be included if requesting the construction, operation, and/or modification of the manufacturing process.

Air Pollution Control Device Forms

Form 3.00 Air Pollution Control Device Information
Must be included if requesting the construction, operation, and/or modification of an Air Pollution Control Device.

Form 3.01 Scrubbers
Must be included if requesting the construction, operation, and/or modification of a scrubber.

Form 3.02 Baghouses and Other Filter Collectors
Must be included if requesting the construction, operation, and/or modification of a baghouse or other filter collectors.

Form 3.03 Electrostatic Precipitators
Must be included if requesting the construction, operation, and/or modification of an electrostatic precipitator (ESP).

Other Forms

Form 4.00 Emissions Information
This form is to be completed and submitted with any Georgia SIP Air Permit application for the construction, operation, and/or modification of process equipment, fuel burning equipment, and/or air pollution control equipment.

Form 5.00 Monitoring Information
Must be included if requesting the construction, operation, and/or modification of an emission unit(s)/source(s) that discharges air pollutants into the atmosphere through a stack(s) and has monitoring equipment installed or monitoring required (Visible emission checks).

Form 6.00 Fugitive Emission Sources
Must be included if the facility has any fugitive emission sources

Form 7.00 Air Modeling Information
Must be included if the request involves the construction, operation, and/or modification of an emission unit(s)/source(s) that discharges air pollutants into the atmosphere generally through a stack(s) and requires computer air modeling.

Additional SIP Permitting Information

Name Change Form
Submit only this form if the facility is changing its name.

Change in Ownership Form
Submit only this form if the facility is changing owners. If a facility name change is also occurring with the change in ownership, do not submit the Name Change Form. The Change in Ownership Form handles both.

SIP Exemption Attachment
Include this attachment with the SIP Application, if the facility has performed any past exempt permitting activities according to Georgia Air Quality Rule 391-3-1-.03(6)(i)3.
If the facility is a Title V source, then an updated Electronic TV Application will need to be submitted
IN ADDITION to the appropriate SIP Name/Ownership Change form. Note: Only the first 3 sections (through the Applicable Rules section) of the E-TV application should need to be filled out to pass the completeness check and create the database file to submit.
